Dental Program

 

Qualifications to receive Dental Treatment:

  • Child must be in elementary school - 5th Grade or under
  • Child has to meet income guidelines for Medicaid or Free Lunch

Who to contact in case my child needs Dental care?

To receive Dental care you will need to contact your local Health Department between the hours of 8am and 4pm.  If a member of the Dental team is unavailable to take your call at that time, please leave a message with your name, phone number, and a brief description of the problem.  If the situation is an emergency make sure that you have contacted a member of the Dental team BEFORE bringing your child to the Health Department so that you can be assured dental treatment.

How will my child get to the Health Department?

During the school year the Dental team will pick up the child during school hours and transport them to and from the Health Department.  You will need to fill out a health history for your child before he/she can be transported.  Health history forms are available from your school.
